Download dgVoodoo2 and apply d3d9.dll to the game directory. The game appears to mishandle DirectX 9 fullscreen transitions on ALT+TAB and this provides some stability. Please use another proxy DLL than d3d9.dll for Gauntlet Enforcement, such as dinput8.dll.
